      Transition is the process students and their families use to think about life after high school, to identify their desired outcomes, and to plan their community and school experiences to assure that the students acquire the knowledge and skills to achieve their goals.

Mission Statement:
The mission of the Tri-County Transition Council is to provide a forum that creates awareness of agencies that are available in our community that will enable young adults with disabilities to have a smooth transition from school to independent living by identifying and communicating supportive resources that maximize opportunities in education and employment.
